This is an electric cooperative service district and apparently they do business different then we do here in Southern Oregon. I am going to be generous and assume that this was a contractor job and being 3 hours from their main service center they arrived and discovered that they were short a few items. This is extremely heavy Winter snow area for this Mountain pass.

The Pinco unipart being used as a standoff insulator but upside down? It will eventually fill up with moisture from rain/snow and freeze resulting in expansion of the insulator and a failure.
