In extreme conditions, depending on how thick the ice is between the top and bottom of the transformers' fuse cutouts (lower crossarm on the pole) and depending on their fuse rating, I have seen them blow in instances like this on 13.8 kv. These kind of outages also occur somewhere after the fuses such as at the transformer bushings, since the metal transformers are grounded.

The installation shown is at a lower primary voltage, I understand is 4 kv.
