The 125 kV pole in this picture dates 1930s and give us a good idea about how a problem has been treated and resolved: One of the three line conductors has been fixed directly on the pole structure, to balance the mechanical loads of the other two. The weight of the 3rd crossarm and its chain is pratically irrelevant. Insulators type FIL 1434. the line got out of service 40 yrs ago circa.
